RHS revised Form RD 3555-21 “Request for Single Family Housing Loan Guarantee” (Form). Effective November 1, 2018, all loan packages must include the revised Form (revision date October 2018), and USDA will discontinue use of the current Form (revision date June 2016). The revised Form will be posted to the USDA eForms and Guaranteed Underwriting System (GUS) websites on October 1, 2018.
