RHS issued a Procedure Notice (Notice) revising forms RD 3555-11 and RD 3555-16. Form RD 3555-11 (Lender Record Change) was revised to remove Block B by eliminating “New Servicing Lender Information” to align with current electronic reporting capabilities and by modernizing workflows to clarify and update the process for reporting a servicer change. Form RD 3555-16 (Agreement for Participation in Single Family Housing Guaranteed/Insured Loan Programs of the United States Government) was revised to update Section B (General Requirements for the Lender), Section C (Underwriting Requirements), Section D (Servicing Requirements), and Section G (Disclosure and Certifications Regarding Debarment, Suspension, and other Responsibility Matters). All revisions are effective July 28, 2026.
